We take our brokers' personal and corporate privacy very seriously. During the course of processing and reviewing your recertification, we accumulate nonpublic personal information from you and/or your company to make an informed decision about recertifying you or your company as a broker with JMAC. We restrict access to nonpublic personal information about you to those employees who need to know that information to review and approve your application. We maintain physical, electronic, and procedural safeguards that comply with federal regulations to guard your nonpublic personal or corporate information. 

We collect nonpublic information about you or your company from the following sources: 
(i) information we receive from you on recertifications or other forms;
(ii) information about your transactions with us, our affiliates, or others; and 
(iii) information we receive from reporting agencies. 

We do not disclose any nonpublic information about our brokers or former brokers to any third party, except as permitted or required by law.

Please provide all authorized contacts and indicate their roles in your company. Below are the definitions for each role:

  • Broker is the Broker of Record, as required under some state mortgage broker licensing requirements, is  the primary loan originator of the brokerage; and is, generally, the manager of all Loan Officers within the organization.
  • Principal Officer is the primary officer or loan originator, as required under some state mortgage brokerage licensing requirements and is the qualified individual for the organization, if required.
  • Authorized Signer is the person(s) authorized to execute any and all agreements and document for your company.
  • Loan Officer is a person licensed as a mortgage loan originator under state or federal SAFE Acts. NOTE: If a Loan Officer processes their own loans, the processor role should be marked as well.
  • Processor is responsible for preparing and organizing loan files for approval but that does not necessarily carry a mortgage loan originator license. A processor may not originate loans without a SAFE Act mortgage loan originator license.

By signing this recertification you agree that JMAC may, at its discretion, provide information to you and/or your company via fax, telephone, and/or email using the phone numbers and email addresses provided herein (or to other telephone, fax, and email addresses as provided by you from time to time) and/or by courier, U.S. postage, the internet, computer, and/or other means, You agree to provide JMAC with updated fax, telephone, and email addresses in order to receive pricing, rates, available programs, specials and other topics and to keep you aware of product opportunities and changes in the marketplace and/or how JMAC does business. You also agree to provide JMAC with annual updates of company financials and licenses. 

It is understood and agreed that documents hereunder may be provided by the parties or may be
executed by the parties electronically, in compliance with JMAC's policies and
procedures relating to e-signatures, the federal Electronic Signatures in Global and National Commerce Act (E-SIGN) and, if applicable, the Uniform Electronic Transactions Act (UETA) adopted by the state in which the electronic record (i.e., the contract or other record created, generated, sent, communicated, received or stored by electronic means) relating to such document or disclosure is initiated.
